About Our Practice

We are a community-centered health home (CCHH) that provides high quality health care services while also working to understand underlying factors that may contribute to your injury or illness.

What sets us apart is our approach to helping you optimize your health as you define it.

 We are focused on treating all of you, not just your immediate medical needs. We want to get to know you, your family, your environment and your community. We are a nonprofit clinic so all profits made are reinvested into providing wellness education, stress management workshops, nutrition classes, weight loss programs and life skills trainings for our patients and their families. It will also help those living with chronic bleeding disorders by providing much needed programs and services.

My Neighborhood Primary Care was started by the Arizona Hemophilia Association to better integrate primary care with chronic care management. It has grown to include all who seek a more integrated, holistic approach to healthcare. Traditionally, the healthcare system is very fragmented, oftentimes with a disconnect between primary care and specialists. The patient is referred to a specialist but there is little to no follow up on how things went, if you are still having any issues, etc. Our approach is to provide integrated and comprehensive healthcare with wrap-around services to treat the whole person, physically and mentally. We offer a patient-centered medical home approach where there is a partnership between the provider, the patient and the family. We partner with you to help connect you to the right specialists, partner with you and your specialists, and reach back out to you to close the loop and ensure continuity of care.

We believe that all people should have their health managed holistically (comprehensive primary care, physical health, mental health, health education and prevention). We want to not only meet your immediate healthcare needs, we strive to help improve the community in which you live, the environment around you, and your internal stressors. We want to get to know you, partner with you and help you optimize your health as you define it.
